Safety concerns led to review at St. Charles Bend

Concerns over patient safety at St. Charles Bend led state officials to conduct a four-day investigation of the hospital this week, but investigators left town finding no major deficiencies or concerns.

A patient suicide at the hospital in February and a death due to a medical error in May....
