McDonald’s of Bend (Pinebrook): 72 — Cooked folded eggs, bacon and Canadian bacon in direct contact with shell eggs in front of reach-in. Many items including cut tomatoes, corn salad topping, clarified butter and pasteurized egg white past date mark. Items including shredded cheese, cheese slices, cut lettuce, hydrated onions and fried onions held at room temperature not marked with time removed from refrigerator. Employees unable to produce quat testing strips for testing concentration of quat solution used to sanitize dishes. Hand sink at dish pit with grill gloves and dirty towel in sink. Hand sink at dish pit does not have paper towels. Spray bottle with chemical not labeled. Wiping cloths in solution containing no residual chlorine sanitizer.
Okawa Steakhouse and Sushi: 78 — Zero parts per million chlorine measured on test strips after final rinse. Noted employee open personal foods and beverages throughout the facility. No paper towels at hand sink near prep sinks. Several spray bottles not labeled.
Active Culture Natural Foods Cafe: 80 — Dishwashing employee had bandage on finger and did not wash hands; employee should wash hands and then wear gloves. Raw shell eggs stored over sliced cabbage and other foods. Chili in hot holding unit at 115 degrees. No quat sanitizer in three-compartment sink (container empty), use to be discontinued until sanitizer provided, dishmachine available. Employee hooded sweatshirt hanging from storage rack with food containers.
Chi Chinese and Sushi Bar: 81 — Raw pooled eggs are stored over dessert in two-door refrigerator. Open can of coke in kitchen. No quat test strips available. No paper towels at back hand sink. Baseball hat, shirt and jacket are stored on dry storage rack holding food items. Cookline floor under equipment has buildup of grease and food debris.
D and D Bar and Grill: 82 — Cookline reach-in cooler at 47 degrees. Corn beef date marked nearly four weeks prior to inspection in walk-in cooler, a tub of cream cheese not date marked but moldy in walk-in. Open cup of coffee in kitchen.
Health inspections were conducted by the Deschutes County Public Health Department and reflect violations observed at inspection. The violations can range from general sanitation and maintenance to violations that have a direct connection to preventing foodborne illness to violations in procedures by maintenance that control the risk of foodborne illness, such as documentation, labeling, personnel training and equipment.
