Bos Taurus: 73 — Raw eggs and batter with raw eggs is stored over chocolate dishes in reach-in cooler. Raw repackaged beef is stored over fruit puree in freezer. Facility does not have department approval of reduced oxygen packaging. Facility has non-frozen fish in reduced oxygen packaging. No chlorine sanitizer is dispensing at kitchen dishmachine. Nalgene and Hydro Flask bottles in kitchen. No chlorine test strips available. No soap available at bar handsink. Wood spoon in kitchen.
Kayo’s Dinner House: 77 — Observed container of one dozen duck eggs stored above cooked desserts in walk-in refrigerator. Observed cooked corn with expired date mark in walk-in refrigerator. Observed heavy cream and half-n-half without date-marking.
Observed multiple foods in front reach-in refrigerator, including cooked chicken, cooked shrimp, cream frosting and open roll of soft cheese, without date marking.
Hola 4 (Bond Street): 85 — Employees wiping hands on cloth towel on shoulder. Repackaged frozen raw shrimp is stored over ready-to-eat foods. Yellow and gray lemon presses are painted and chipping, not restaurant-approved.
Mantra Indian Kitchen: 85 — Chicken curry is 60 degrees after cooling overnight in large plastic container. Cooked shrimp and chicken are vacuum-packed (three bags). No chlorine available in bar dish-machine. Wood muddlers used at bar also losing varnish coating. Mold build-up in ice-machine interior.
The Wallow Bar and Grill: 85 — Raw bacon stored over ready-to-eat ham. Ice bath not up to the level of the products for salsa, cheese, shredded lettuce. Reach-in refrigerator to the right of the line holding at 45 degrees on bottom. Cowboy beans and cheese sauce past date mark. Items not re-dated when pulled from the freezer, milk in walk-in not dated, prepped tomatoes not dated. Ice mold growing on baffle of ice machine.
Health inspections were conducted by the Deschutes County Public Health Department and reflect violations observed at the time of inspection. The violations can range from general sanitation and maintenance to violations that have a direct connection to preventing foodborne illness to violations in procedures by maintenance that control the risk of foodborne illness, such as documentation, labeling, personnel training and equipment. During and after inspections, food vendors were allowed to correct violations to improve scores prior to a follow-up inspection.
