Black Bear Diner (Redmond): 79 — Observed plastic wrapper in bulk sugar. Raw chicken over raw beef in walk-in cooler. Raw egg next to oranges in walk-in cooler. Slicer and dicer in prep area are dirty and have not been used for over 24 hours. Ranch, ham and pie crumbs were eight days old.

No paper towels at hand washing sink in prep area. Knives in the kitchen were stored between cold hold and hot hold units. Thermometer in the beverage air unit was not easily readable. Hand washing sink is leaking.

Hola 2: 79 — Repackaged, portioned beef in chest freezer not fully frozen on top of vegetable-based frozen foods.

Beans and chicken cooked the day earlier in walk-in at 51 degrees in center. Cheese in line cooler holding 50 degrees in stacked container. Water bottle stored on top of the line cooler. Two spray bottles not labeled.

VFW Post No. 1643 (Ponderosa Pine): 81 — Observed containers of to-go dips and veggies with expired sell dates. Observed concentration of chlorine in wiping-cloth bucket measured greater than 200 ppm. Observed cut tomatoes, bag of cooked bacon and cut iceberg lettuce in reach-in cooler without date marking. Observed no chlorine test strips for use with wiping cloth sanitizer solution. Observed bags of potatoes and onions on floor in dry-storage room.

Mazatlan Mexican Restaurant (Redmond): 82 — Chili rellenos were at 99 degrees in steam table. Multiple food items above 41 degrees in cold top on cook line. Ware washing machine was below 50 ppm chlorine sanitizer. Spray bottle with clear liquid with no label. Chlorine sanitizer buckets were below 50 ppm. Observed latex gloves in food-service establishment.

Super Burrito: 82 — Glass reach-in cooler is holding food at 46 degrees, salsa bar with cooked carrots/peppers at 47 degrees. Bottle of acetaminophen stored over prep table. Front hand sink has roll of non-perforated paper towels at hand sink with no dispenser. Wooden spork in kitchen. Cheese is repackaged in unapproved grocery bags. A covered receptacle is not provided in the female restroom.

Health inspections were conducted by the Deschutes County Public Health Department and reflect violations observed at the time of inspection. The violations can range from general sanitation and maintenance to violations that have a direct connection to preventing foodborne illness to violations in procedures by maintenance that control the risk of foodborne illness, such as documentation, labeling, personnel training and equipment. During and after inspections, food vendors were allowed to correct violations to improve scores prior to a follow-up inspection.
