E BarGrill: 81 — Clam chowder cooling in walk-in at 150 degrees. Cooked sausage in refrigerator is expired. Some ready-to-eat foods are not properly date marked. No asterisk on breakfast menu for eggs that can be cooked to order. Ice machine has a layer of mold-like substance inside. Back side of meat slicer has encrusted debris. Mold is growing on the walk-in walls and bar reach-in fridge. Box of potatoes on kitchen floor. Meats thawing at room temperature for over 45 minutes. Gaskets on cook line not in good repair.

Racks in walk-in are rusting. Peeler has masking tape wrapped around it. Microwaves in the kitchen are dirty inside and out. Fans guard in walk-in dirty, shelving racks in reach-in, fridge make-up table are all dirty. Goose-neck spray hose is dirty. Steamer is leaking.

Sunriver Carson’s Kitchen/Twisted River: 82 — Raw egg whites held over ready to eat items. Right side of line cooler (saute cooler) holding items at 45 degrees. Items had been in cooler for approximately 2.5 hours. Many ready-to-eat items in walk-in past seven-day date mark. Some items in facility with no date mark when opened, such as milk, cream and whipped cream cans. Observed raw meat in reduced oxygen packaging bags sealed in facility in freezer.

Solomon’s (Tetherow Grill): 81 — Raw chicken over fish on cook line on Solomon’s line. Cut meats (chicken, ham) were above 41 degrees. Shellfish tag with no date of last consumption. Milk product in Solomon’s walk-in not dated and at The Row bar not dated. Foods are held at room temperature without written procedures indicating the time that the food has been removed from temperature control. Raw chicken stored on the floor in both walk-ins. Ice machine and soda gun in Solomon’s Kitchen and bar has a build-up of biofilm. Silverware going through the ware-washing machine for the second time with food contact surface up.

Rio Restaurant (Sisters): 81 — Observed cook talking on cell phone and cooking at the same time, scratching their face. Hand washing sink is leaking water. Observed hand washing at sink where dirty dishes are stored. Observed a cup with no lid, handle or straw. Bulk food in reach-in with no dates (soup, beef, octopus).

Shari’s No. 163 (South): 84 — Multiple items (quiche, split pea soup and fish) in walk-in were past date marked. Atmosphere vacuum breaker is leaking. (Removed hoses until plumbing is fixed.) Fan guard in reach-in on cook line is dirty.

Health inspections were conducted by the Deschutes County Public Health Department and reflect violations observed at the time of inspection. The violations can range from general sanitation and maintenance to violations that have a direct connection to preventing foodborne illness to violations in procedures by maintenance that control the risk of foodborne illness, such as documentation, labeling, personnel training and equipment. During and after inspections, food vendors were allowed to correct violations to improve scores prior to a follow-up inspection.
