Palmer’s Cafe: 67 — Observed employee touched raw burger and cracked raw shell eggs then touched ready-to-eat tortilla without washing hands. Observed plastic container of beef and vegetable soup at 46-80 degrees in steam table, double panned inside metal container. Per operator’s comments, container of soup had been placed from freezer into steam table approximately 1 hour prior.

Observed sliced tomatoes at 54 degrees and sour cream at 47 degrees on ice, and several containers of pooled eggs at 60 degrees. Observed five cases of raw shell eggs in back storage room (open to alley). Ambient temperature in back storage room measured 56 degrees. Observed open bottle of liquid band aid antiseptic and box of band aids stored in back storage room. Observed open containers of ham and tomatillo salsa without date-marking in reach-in refrigerators in kitchen.

Observed container of expired ground beef in refrigerator. Operator stated that the ground beef had been cooked, frozen and then pulled to thaw the day prior. Observed tubes of raw ground beef thawing at room temperature in kitchen. Observed mouse droppings in back storeroom by brown sugar. Observed mouse droppings under front hand-washing sink.

Okawa Steakhouse and Sushi: 81 — Dish machine not registering chlorine. Hood system with build-up of dripping grease over cooking surfaces. Noted open beverages and food throughout the kitchen. Date of use is not recorded on shellfish tags and tags are not kept in chronological order. Sanitizing spray bottles are not labeled.

Jake’s Diner: 82 — Observed temperature of mashed potatoes was 117-138 degrees; brown and yellow gravies were 124 degrees. Observed concentration of chlorine in final rinse of low-temp dish machine measured below 10 ppm. Observed concentration of chlorine sanitizer in wiping cloth buckets measured greater than 200 ppm. Observed German Benedict on board as today’s special contains poached eggs, without consumer advisory. Observed accumulation of grease and food debris on sides of equipment and floor under grease hood, grease on ceiling and walls near cook line.

Baldy’s BBQ (Highway 20): 82 — Observed containers of cheese sauce (made in-house) in walk-in refrigerator with expired date. Observed no detectable chlorine in front-of-house dish machine. Observed no detectable chlorine in back of house dish machine. Observed container of potato salad dressing (made in-house) in walk-in refrigerator without date marking. Observed wet wiping cloths stored on countertops in establishment. Observed broken and damaged lid and sides of ice cream freezer (used to hold fries).

Greg’s Grill on the Deschutes: 85 — Stock in refrigerator for approximately three hours at 100 degrees. Expired topping and ham in the refrigerator. Open milk, open cream cheese and whipped cream are not date marked. Wood boards used for charcuterie platters are not smooth and easily cleanable. Ice machine with biofilm on baffle. Hood in back area not clean; air return over clean dishes not clean.

The Blacksmith Restaurant: 85 — Uncooked diced bacon in prep line reach-in cooler insets. Bacon is immediately next to ready-to-eat foods. Reach-in cooler (pantry) holding foods at 44-45 degrees (potentially hazardous foods removed). No chlorine sanitizer at bar dish machine (repairs called in).

Health inspections were conducted by the Deschutes County Public Health Department and reflect violations observed at the time of inspection. The violations can range from general sanitation and maintenance to violations that have a direct connection to preventing foodborne illness to violations in procedures by maintenance that control the risk of foodborne illness, such as documentation, labeling, personnel training and equipment. During and after inspections, food vendors were allowed to correct violations to improve scores prior to a follow-up inspection.
