Impact Health Inc. has been told to stop forging consumer’s signatures on insurance documents, violating Oregon insurance law, according to the Oregon Department of Consumer and Business Services.

About 1,600 consumers were offered $50 gift certificates for filling out an insurance survey and providing a copy of their health care identification number to Impact Health Inc. through Facebook or Craigslist, according to a statement released by the Division of Financial Regulation.

More than 900 agent of record forms were filed, allowing Impact Health to receive a commission from the insurance company. The division confirmed a dozen signatures were forged.

The division has also issued a cease and desist order against Helen Kyung Lee and Joany Inc., who are affiliated with Impact Health Inc. The division is pursuing fines and seeking to revoke the licenses.

Anyone who filled out the survey should contact the insurance company to confirm that it has the correct agent on file. If the agent on file is incorrect, consumers should contact the division’s consumer advocate at 888-877-4894.
