Editorial: Another failure in foster care

Another child died in Oregon’s foster care system. It was a 17-year-old, living in a hotel room with Oregon Department of Human Services employees.

The child died by suicide on Aug. 3.

There will be a full investigation.

One component of the review will be done by what the state calls a “critical incident review team.” They look for issues that could be corrected to prevent future deaths in the state’s foster care system. The reports they create are public documents.

The reports are difficult to read, even if they are stripped of identifying information. Every report has a terrible ending and shows a cascade of failures.
